WHAT IS MELASMA?
Melasma are blotches of brown skin pigmentation that appear on the face, namely,
cheeks, nose and forehead. In the case of women, sometimes they merge to form
the 'Mask of Pregnancy' which it is otherwise known as Melasma.
MELASMA IN WOMEN
The majority of women suffer from Melasma. Melasma generally occurs during
pregnancy, menopause or on women who are taking oral contraceptives. Melasma is
usually aggravated by sun exposure and may become permanent if not treated
early. It may also recur in successive pregnancies.
MELASMA IN MEN
Only a small percentage of men suffer from Melasma. The probable causes are sun
exposure, but hormonal factors influence it as well, and it is further worsened
by exposure to the sun.
HYPERPIGMENTATION
There are three (3) types of hyperpigmentation:
1) Melasma
2) Freckles
3) Age Spots
MELASMA
Melasma, as medically known as Chloasma, is considered a fairly trivial cause of
skin hyperpigmentation and is typically seen in pregnant women. As many as 50% -
70% of females notice hyperpigmented areas of the skin as pregnancy proceeds.
This type of melasma, known as melasma gravidarum or chloasma gravidarum, is
most commonly noticed as a tan or brownish discoloration on the face, as far up
as the hairline, and extending down to the jawline -- thus, the common synonym:
"mask of pregnancy".
The blotches are often symmetric and confined to the forehead and chin. However,
the hyperpigmentation may also manifest on the nipple area (areola), armpit (axilla),
and genitals. Skin abnormalities such as scars or moles may become darker.
Melasma is more common in dark-skinned persons, and is worsened if the skin is
exposed to sun.
In response to sun exposure, melanin is produced by melanocytes cells, which
contain oestrogen receptors. Progesterone and oestrogen (female hormones
essential for healthy functioning of the female reproductive system) stimulate
melanin formation. Their levels rise during pregnancy, possibly causing mask of
pregnancy (melasma) and may persist for many years. Melasma occurs in 5% - 34%
of women taking oral contraceptives, probably due to the increased hormone
intake.
As many as 10% of patients with melasma are male. Studies show that the probable
cause in males is sun exposure. Hormonal factors also influence it, and it is
worsened by exposure to the sun.
FRECKLES
Freckles are common amongst a lot of people. Some patients have a variety known
as "sunburn freckles", or ephelides. These are larger and darker than standard
freckles, with a medium-brown color. The edges are irregular, but the pigment is
even throughout the lesion. Sun-induced freckles are associated with
non-melanoma and melanoma skin cancers.
AGE SPOTS
Studies show that age spots are caused by sun exposure. Since it is cumulative
exposure that produces these discolorations, they are more common in older
people, and are found on sun-exposed areas such as the face and forearms. They
do not fade without treatment, regardless of the degree of sun protection taken.
NEVUS - A SKIN BLEMISH
THERE ARE TWO TYPES :
Melanocytic (pigmentated) Nevi - is caused by abnormality or over activity of
skin cells producing the pigment melanin. E.g. freckles which are small, flat,
light to dark brown found on skin areas exposed to sunlight.
Vascular Nevi - is caused by an abnormal collection of blood vessels.
MELAMIN
The pigment that colors the skin, hair and the iris of the eyes.
HOW SERIOUS IS SKIN
DISCOLORATION
The skin is the largest organ of the human body. Because it is easily visible,
any areas of color change show up easily. Many changes are minor, such as "age
spots." However, some changes can be serious. An annual examination by a
dermatologist is recommended to detect the early signs of skin cancers. As
mentioned earlier, several types of skin abnormalities causes pigmentation. They
could be sun exposed areas causing freckles, age spots, melasma or other skin
abnormality such as scars due to surgeries, outbreak of
pimples or accidental cuts, which causes
discoloration.
Fortunately, these discolorations or pigmentation are treatable by Drula Cream

ABNORMALITIES OF PIGMENTATION
Patches of dark skin mingled with lighter areas may follow an episode of eczema
or psoriasis, or they may occur in tinea versicolour. In Chloasma, hormonal
changes cause dark areas to develop on the face, this may occur in woman taking
oral contraceptives, or during pregnancy or the menopause. Some perfumes and
cosmetics may also cause dark facial patches, particularly when they contain
chemicals that cause photosensitivity. All such patches of discoloration usually
fade with time.
Permanent areas of pronounced deep pigmentation are usually due to an
abnormality in the melanocytes, as is the case with moles and freckles.
